Presentation to the Regional Community of Practice (CoP)

Our UBC IDEA Lab research team shared the findings and real-life experiences of our residents, families and staff from the “Virtual Team-Based Care Planning for Transitions in Care” project with the Regional Community of Practice (CoP). The conversations were fantastic and meaningful.

The focus was on adopting a framework to address the unique needs of individual older people, their families, and staff during the virtual care journey through a holistic, team-based, person-centred approach. This approach facilitates inclusive care planning and ongoing quality improvement momentum.
