On October 30, Dr. Lillian Hung and our older adult partner, Dr. Jim Mann, LL.D., co-presented at the Canadian Institutes of Health Research | Instituts de recherche en santé du Canada (CIHR) Policy Research Roundtable 2024, shedding light on our project, Virtual Team-Based Care Planning for Seamless Transitions in Care. This initiative addresses delays in hospital discharges for older adults with dementia, aiming to enhance patient safety, quality of life, and health equity.

Through policy mapping, interviews, and focus groups, our team has identified essential strategies for optimizing care transitions.

The project aligns with the Quadruple Aim, striving for better care, improved health, cost reduction, and enhanced caregiver experience.

Thank you to the support of CIHR and Vancouver Coastal Health (VCH), our team members from the University of Victoria, Western University, McMaster University, VCH, PIECES Canada, UBC IDEA Lab and all participants for their invaluable insights. Together, we’re advancing the future of inclusive virtual team-based care of our older community.
